  • **We respect your privacy and will never share your information without your consent. We use email as a form of primary communication to relay important information or changes about our hospital, education and share our upcoming promotions. In addition, you may receive your pets healthcare reminders and have the ability to confirm upcoming scheduled appointments.**

  • Treatment Consent

    I, the owner of the patient(s), hereby authorize the veterinarians and staff of Mountain View Animal Hospital & Holistic Pet Care to examine, treat, and prescribe necessary medication for the above listed pet. This includes, but is not limited to, procedures therapeutically and/or diagnostically. I understand that no guarantee of successful treatment is made. I further understand that this facility has a doctor on staff between 7:30am to 5:30pm only, therefore, there is no continuous presence of doctors after regular business hours. I certify that all the information above is correct, that I am the rightful owner or authorized agent of this pet(s) and that I am at least 18 years of age. Mountain View Animal Hospital & Holistic Pet Care does not operate as an emergency hospital and although we will always try to be available, I understand I may be referred to an emergency hospital even during open hours if we feel it is best for my pet(s).

    I understand by Nevada pharmaceutical law, all prescriptions including herbals that leave the hospital are non-refundable, even if my pet(s) has a reaction.

    Payment Policy

    Please note: All fees incurred at Mountain View Animal Hospital & Holistic Pet Care are required to be paid upon time of service. We do not accept payment plans or delayed billing. Any unpaid balance left unpaid are subject to accrued 1.5% interest after 30 days, and balances left unpaid for more than 90 days will be sent to collections in addition to a 40% collection fee. There is a $35 charge for each returned check, and we do not accept any postdated checks.

    We accept Visa, Mastercard, American Express, CareCredit & for established clients (been here at least 2 times) we also accept checks.

    Cancellation Policy

    We request that any cancellations be made at least 24 hours prior to the start of your pet's medical appointment, grooming appointment or boarding/daycare reservation. If you cancel without ample notice or do not show up to your appointment or boarding/daycare reservation you may be required to pay a cancellation fee. This fee is non-refundable and cannot be applied toward any other services.

    At Mountain View Animal Hospital and Holistic Pet Care, we LOVE to share our patient's cute faces! We try to photograph their success stories, "spa days", playtime in boarding or daycare, and really any moment we can capture that expresses that love! In addition, we add your pet's picture to their medical profile for safety and identification purposes. With this, we require authorization.

    I hereby grant Mountain View Animal Hospital and Holistic Pet Care permission to use any photographs taken of my pet in all its publications, including website and social media entries. I understand this is voluntary and I am aware I will receive no compensation. I understand and agree that these materials will become property of the hospital. I hereby release the hospital and its employees from any claims, demands, and causes to action which I, my heirs, representatives, executors, administrators or any other persons acting on my behalf or the behalf of my estate, have or may have by reason of this authorization.

  • Release of Liability for Holding

    Our staff is trained to physically manage, hold, or otherwise restrain your pet in the safest and most comfortable way. Should you decide that you wish to assist during exams or procedures, our staff and hospital shall not be held liable for any injury that may arise, whether it be done to or by your pet.

    I have read and understand the release of liability for holding my pet and the media consent form and have been given opportunity to ask any questions.

  • Thank you for choosing Mountain View Animal Hospital and Holistic Pet Care (MVAH/HPC) to provide boarding and/or daycare services for your pet(s). We strive to provide a safe, clean, and fun environment for your pet(s).

    In order to accomplish this, we ask that you acknowledge and agree with the following information, terms, and conditions of this agreement

  • Disclaimer: Although we are proud to be associated with a medical facility, please note that your dedicated pet resort team is separate from the hospital and are not trained medical professionals. Doctors may be alerted if there is major illness noted but do not perform daily rounds. 

  • Evaluation: I understand that if I would like to have my pet(s) participate in daycare and/or boarding that they must have an evaluation. If you are a current patient at MVAH/HPC the evaluation fee and first day fee will be waived. If you are not a current (seen within the past 12-months) patient of MVAH/HPC there will be a $41.50 fee for evaluation and the first day fee will be waived. Our evaluation is a comprehensive system of interaction with varying types, sizes, and ages of other pets that we have permission to interact with as evaluators. Due to the nature of interactions between dogs there is always a risk of injury. It is mutually understood that any injury to your pet will be your responsibility for all costs associated with the treatment from said injury.

  • Pickup/Drop Off: Monday through Saturday, from 7:00 am until 6:00 pm. We cannot accept any after hour pickups or drop-offs due to facility security and the safety of your pet and our team,

    *Pet(s) are available for pick-up on Sunday. We just ask that you prepay due to the main part of the hospital being closed, thus we are unable to accept payments.

  • Payment: We ask that payment is made prior to, or at the time of drop-off. There may be charges for unforeseen emergency services required during my pet(s) visit. These will be invoiced to you upon your return. We do offer payment over the phone by debit or credit card. You can call us at (775) 853-6900 to access that option. 

    Food/Medication(s): Please provide enough food and medication for your pet when checking in with our Pet Resort team for the duration of their stay, plus one day. Medications are very important to us in order to keep your pet healthy. Please provide all medication in original prescription containers.

  • Vaccinations: For the protection of your pet, and all our boarders, we require all the following vaccinations:

    Dog: Rabies, DA2PP, Bordetella and Influenza

    Cat: Rabies and FVRCP

  • *We do recommend that your pet(s) be on a flea preventative medication prior to being boarded and have a negative fecal on file. Although we take every precaution to prevent internal and external parasites, the best protection for my pet(s) is a regular preventative schedule as directed by a veterinarian. If my pet(s) is found to have internal or external parasite(s) while boarding, he/she will receive treatment by one of our outstanding veterinarians; and, be charged in addition to the boarding/daycare costs for the treatment.

    *We also recommend that you provide any relevant medical records if your pet has a chronic illness or injury.

  • Existing patients of MVAH/HPC will have their fee for examination waived. If you are not an existing patient with MVAH/HPC, then all charges will apply for said treatment.

  • Bedding: Mountain View Animal Hospital will provide all bedding for my pet(s). I understand that I can bring one comfort item that fits into my provided MVAH/HPC to go bag. Please understand that it is impossible to track who brought the same red ball for their baby to play with. Therefore, we cannot be held responsible for the loss or damage of any items brought from home. It is our policy to immediately remove any comfort items found in disrepair so that we can ensure your pet(s) safety. MVAH/HPC will not be held responsible for any medical issues arising from ingestion and/or misuse of comfort items which you have provided for your pet(s).

    Pool: During high temperatures of summer we (please select below agree/decline) to have our pet(s) access our pools to cool off and play. I understand that the pet resort team will take care of my pet after swimming by drying them off. However, they may roll in dirt and not be as clean as when you dropped them off for boarding or daycare.

  • Arts/Crafts: From time-to-time, we have arts and crafts projects with your pet(s). I (please select below) do - or do not, authorize my pet(s) to participate. I understand water based non-toxic paints may be used and may leave washable residue on my pet.

  • After Hours: MVAH/HPC is not a 24-hour boarding facility and my pet(s) will be without Pet Resort supervision from the hours of 9:30 PM - 6:00 AM. In addition, our physicians are available on-site only Monday through Saturday from 7:30 AM - 5:30 PM. In an emergency my pet may be transferred by a team member of MVAH/HPC to Animal Emergency Center located at 6425 South Virginia Street, Reno, NV 89511. I authorize said MVAH/HPC to transport my pet to this facility if a specialized treatment, diagnostics, or continued monitoring will be required.

  • I have read and understand MVAH/HPC's boarding and daycare agreement. I confirm that I am the legal owner of said pet(s). I waive and release MVAH and its staff from all liability of any nature which includes death, sickness, or damage to my pet resulting from any daycare and/or boarding.
