Thank you for your referral and trust in Mountain View Animal Hospital and Holistic Pet Care. Our intention is to serve your client with utmost professionalism. We currently accept referrals for our Holistic Services and Orthopedic Surgery Services . For each patient referred, we kindly request pertinent medical records and a completed Veterinarian Holistic Referral Form or Orthopedic Surgery Referral Form.

Once the Veterinarian Referral Form and pertinent previous medical records are received, we will reach out to your client within 2-5 business days.
